Important Notes

Practice in Wyoming

  • No person may practice as a Dentist or Dental Hygienist in the State of Wyoming without first obtaining a license from the Board.

Applying for Licensure

  • The Board office will not communicate with any individual except for the applicant regarding the status of any application.
  • The Board will NOT accept cash.

Dentist Application Forms

Dentist by Examination (New Graduates) - For new graduates that are not yet licensed in any jurisdiction.

Dentist by Endorsement - For those actively licensed in another jurisdiction.

Dentist by Endorsement w/ Temporary Educator's Permit - For those actively licensed in another jurisdiction and requesting a temporary educator's license to instruct at a dental hygiene school.

Non-Clinical Dentist - For those seeking a limited license for teaching, examining, consulting, reviewing, or engaging in administrative oversight.

Sedation Application Forms

Moderate Sedation Permit - To administer Moderate Sedation

Deep Sedation/General Anesthesia Permit - To administer Deep Sedation and/or General Anesthesia

Facility Permit - If a permit holder chooses to allow an operating dentist to utilize their facilities to perform dental procedures, then the permit holder shall apply for a facility permit.

Operating Dentist Sedation Permit - An operating dentist sedation permit shall be required for an operating dentist to perform procedures where sedation services are provided by a qualified anesthesia provider.

List of Approved Clinical Inspectors - To view individuals approved by the Board to perform inspections

Onsite Clinical Re-Inspection - To verify completion of office re-inspection (due every 5 years)
