Renewal Forms & Instruction
The 2023-2024 renewal period is open.
Instructions - Please read!
Do not mail or bring in cash.
Please do not call to confirm receipt of your renewal. Due to the volume of renewals received, staff is unable to go through documents to confirm receipt of a specific renewal. If you need to ensure we received your mailing, send it via a trackable method.
From date of receipt, renewals are generally processed in 3-4 weeks. If you don't yet have your renewal cards and wish to see if your renewal was processed, please go to https://dental.wyo.gov/public/lookup to see if your license shows as renewed. If you need your renewal documents in hand prior to the end of the year, you must submit your renewal docs prior to December 1.
Continuing education is due by December 31, 2023. Licensees will be asked to simply attest to their CE completion on their 2023 renewal. Staff will conduct a random audit of renewals following each renewal period. If you are selected for random audit, you'll be asked to provide your CE certificates at that time. You can find additional continuing education information HERE or under the "Licensing" heading above. This will answer most questions you have. Please read before contacting the Board Office for additional information!
Assistant Renewal Forms
Effective 2022, radiograph and pit and fissure permits must be renewed separately. A complete form and fee is required for each permit.
Late Renewal Forms
Dentists & Hygienists - acceptable January 1, 2024 through March 31, 2024.
Assistants - acceptable if your permit is expired within last five (5) years.

Dentists & Hygienists
Dental Assistants
If you hold both a radiograph and pit and fissure permit renewal, they must be renewed separately according to the Rules changes effective January 14, 2022. If your permit expired five (5) or more years ago, you must apply as if you were new to the state.

Dentists:
You must apply for "Dentist Relicensure" which costs $370.
Hygienists:
You must apply for "Hygienist Restoration" from April 1, 2024 through December 31, 2024 which costs $190. This will restore your license through December 31, 2025.
You must apply for "Hygienist Relicensure" from January 1, 2025 and on which costs $190.
