License holders who do not renew by December 31 may renew during the administrative grace period from January 1st to March 31st following the renewal period, however, may not practice until the renewal is received.
After March 31st, please use applicable Restoration and Relicensure applications below.

If you hold both a radiograph and pit and fissure permit renewal, they must be renewed separately according to the Rules changes effective January 14, 2022. If your permit expired five (5) or more years ago, you must apply as if you were new to the state.

If you do not renew your license by March 31, 2026 you must apply for "Dentist Relicensure" and submit the required relicensure fee of $370.
If you do not renew your license by March 31, 2026 you must apply for "Hygienist Restoration" from April 1, 2026 through December 31, 2026 which costs $190. This will restore your license through December 31, 2027.
If you do not restore your license by December 31, 2026 you must apply for "Hygienist Relicensure" and submit the required relicensure fee of $190.
